Care Plan Ending
The care plan ending report allows you to see what care plans are coming to an end on a patient’s case, in a specific date range.
This helps monitor care and when a reevaluation may need to take place to determine if a patient need to change the type of care they are on or be placed on a new care plan.
This Report generates by Assigned Provider. If a Provider is not Assigned to the patient, in their patient file> Patient info tab, they will be seen under the Unassigned category.
To Run the Report
Under Reports
Care Plan Ending
Select Appropriate Filter:
Care Plan with __ or Fewer visits left and ending in the following date range, add the number of Visits and Date Range
Or
Care Plan with an end date that falls in the following date range and then select Date Range.
Options for date range are: Range of days, Day, Week, Month, Quarter or Year
Select Provider:
the software defaults to ALL providers but, you may select a specific Provider.
Here is a sample of what the report will look like:
You will see the provider the patient is assigned to, name of the patient, case, care plan name, scheduled end date, number of remaining visits if additional visits were included, and next scheduled appointment date.
To Download/Print a Report
All reports will display a toolbar at the top of the page when generated.
You will be able to Print and Export the document. You will see the Red Circle is highlighting the Print button and the Red Arrow pointing to the Export options. X=Excel, A=Adobe and W=Word.
