Here are some reasons why you might receive this error:
- Cpt Code is not included in the Fee Schedule placed into the Insurance Policy
- Start Date for CPT Code in Fee Schedule is not backdated to cover the dos (Date of Service) you are attempting to attach it to.
- When using OTC (over the counter), any CPT code you would like to apply to the patient ledger, must be in fee schedule w/ Appointment required box unchecked.
- The CPT code shown on the Fee Schedule is not an EXACT match to the CPT code and Description used to create the CPT code button.
Follow these steps:
- Cpt Code is not included in the Fee Schedule placed into the Insurance Policy
If a patient has a Fee Schedule attached within their Insurance Policy, you will want to check the Fee Schedule to make sure the CPT code you are attempting to attach is present.
First, find Fee Schedule within Insurance Policy:
Next, let’s pull up the Fee Schedule
Now, let's check the Fee Schedule attached to see if the CPT code you are trying to Charge is on it:
Choose the correct Payer and Fee Schedule Attached
If the CPT code is not on this fee schedule, it must be added.
To add a new CPT code to an existing Fee Schedule, please CLICK HERE
2. Start Date for CPT Code in Fee Schedule is not backdated to cover the dos (Date of Service) you are attempting to attach it to.
In the Fee schedule, choose the appropriate filters needed and pull up the fee schedule you are wanting to change the Cpt Code Start Date on . Keep note of the Cpt Code you are wanting to change and scroll all the way to the right. Click on the Start Date and a drop down will populate and enter in the date of service you want the charges to start.
Click: SAVE when done.

The Image below shows how one CPT code can have numerous descriptions. When following the flow of adding and choosing the right CPT code and description preferred to create the button, the EXACT same one needs to be added to the price list and fee schedule. If another one is chosen the button will be broken.
**If you're unsure of the proper code and description used to create the button, you may need to consult the original author of the button structure or look at a price list or fee schedule where it will display the correct code and description used, when the button works properly.**
